We have been trying to ‘engage’ the patient in their own healthcare, initially providing services in home, then remote care monitoring and chronic care management, then a hospital @ home initiative. None of these efforts have been successful in keeping the patient engaged. Even the most enthusiastic patients get discouraged after a few months.

The ONLY topic the patients seem to ‘engage’ in is medication management. They rely on physicians and nurses to take care all other aspects of care except taking, organizing and reordering medication. We believe this is a way to engage them in their own healthcare.
